Reduction and Avoidance of Medication Errors in Senior Living
November 26, 2024Tuesday, January 28, 2025 at 1:00 pm ET
Presented by: Caroline Garvey, Manager of Clinical Operations, PharMerica & Al Buchta, Risk Management Analyst, Caring Communities
The number of errors in medication management in assisted living is increasing, placing both residents and communities at risk. Common errors include incorrect dosage, medication or resident, missed doses, inadequate monitoring and challenges with medication availability. The risk of these errors can cause resident harm and create risks for communities.
Further complicating the already ever-present risk for errors is an increase in factors such as acuity creep, staff shortages, lack of knowledge, and organizational challenges. Fortunately, there are steps communities can take to minimize and avoid them to protect their residents and staff.
In this session, we’ll discuss real-life scenarios and the errors that occurred along with the impact to buildings. We will wrap up by covering recommended steps and systems communities can put in place to prevent mistakes going forward.
Learning Objectives
- Understand the impacts of medication errors on a community and its residents
- Learn the various factors that contribute to medication errors
- Hear case studies featuring medication errors and what could have been done to prevent them
