This program will familiarize the audience with clinical challenges faced by people with intellectual and developmental disabilities (I/DD) and discuss how the use of value-based outcome measures and home-based primary care (HBPC) may improve outcomes in this vulnerable population.

Upon completion of this presentation, attendees should be able to

1. Describe the clinical conditions that people with I/DD face.
2. Define and describe emerging value-based outcomes measures in populations of individuals with intellectual and developmental disabilities (I/DD).
3. Describe how a home-based primary care model works.
4. Develop an understanding of home-based primary care’s effect on clinical outcomes among individuals with I/DD.
