More chronic conditions, legacy prescribing, and complicated prescription regimens are all contributors to polypharmacy. And it can cause significant issues for seniors, from drowsiness and confusion to falls and general decline. Ensuring the appropriateness of each medication is critical to optimal health outcomes among residents, but it can be challenging for nursing teams to implement an effective process, especially amid high rates of staff turnover.
In this session, we’ll discuss the importance of collaboration between DONs, pharmacists, and prescribers for optimal medication regimens, tips for developing a step-by-step process for deprescribing and how to monitor it for success, and ways to overcome potential challenges like a resident’s refusal to change their medications. Attendees will leave prepared to create an effective process for their facilities.

2. Explain the roles of the DON, pharmacist, and prescriber.
3. Identify how to set a facility up for success through training.
