This program will introduce attendees to the value-based care landscape in long term care. The clinical phenotype and cost profile of long-term care residents will be discussed, and the impact of these beneficiaries on the US healthcare system and Medicare program will be explained. We will conclude by describing emerging value-based care opportunities in the long-term care and senior living community space.
Upon completion of this presentation, attendees should be able to:
- Describe what a value-based care model is
- Describe the chronic condition and cost profile of a long-term care and senior living community resident
- Name and describe the basics of the ACO and ISNP model
- Describe opportunities for long-term care and senior living community residents and care teams in value-based care models
